Endoprothetik bei Patienten mit Osteoporose

Zusammenfassung

Die Endoprothetik ist zu einer äußerst erfolgreichen Operationsmethode mit hervorragenden Langzeitergebnissen geworden. Die gezielte Diagnostik zum Ausschluss einer Osteoporose vor einem Gelenkersatz und der Einsatz von Medikamenten zur Verbesserung des Ergebnisses bei Gelenkersatz können derzeit nicht empfohlen werden. Im Bereich der Knieendoprothetik sollte bei bekannter Osteoporose immer zementiert werden. Gleiches ist für die Hüfte zu empfehlen, auch wenn für die acetabuläre Komponente zementfrei gute Ergebnisse erzielt werden. Die zügige Belastung und Mobilisierung erscheinen vorteilhaft, auch mit Blick auf die Osteoporose und ihre systemische Behandlung.

Zur Zementierung einer Knie-TEP wird in Deutschland zu über 98% Knochenzement verwendet, der mit einem Antibiotikum beladen ist. Ob er wirklich besser ist als Zement ohne Antibiotikum, kann laut Registerdaten bezweifelt werden.
