Zusammenfassung

Die Endoskopie ist die führende Methode in der Diagnostik gastrointestinaler Malignome. Trotz zunehmender Vorsorgestrategien liegt bei steigenden Inzidenzen verschiedener Tumorentitäten bei zahlreichen Patienten schon eine palliative Therapiesituation vor. Die palliative Endoskopie kann hier maßgeblich zur Linderung tumorbedingter Symptome wie Dysphagie, Malnutrition, Cholestase, Ileus und Schmerz beitragen. Hierzu können verschiedene Ansätze zur Sicherung der Ernährung (perkutane endoskopische Gastrostomie, perkutane enterale Jejunostomie), der Aufrechterhaltung der Magen-Darm-Passage (Stents) und des Sekretabflusses oder zur Ableitung gestauter Hohlorgane angeboten werden. Endosonographisch gestützte Punktionen bieten darüber hinaus die Möglichkeit, bei fehlender Erreichbarkeit oder fehlender Passierbarkeit von Tumorstenosen durch Stenteinlage neue Abflussmöglichkeiten oder Kontinuitäten zu gewährleisten. Bei allen Interventionen sind mögliche Kontraindikationen und Komplikationen zu berücksichtigen.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
