As computed tomography (CT) imaging pushed the frontiers of early lung cancer detection, it uncovered a minefield of ground-glass or pure non-solid nodules, previously camouflaged in conventional chest radiography. These hazy silhouettes, regularly less than 1 cm, often harbor indolent yet potentially dangerous adenocarcinoma in situ (AIS) or minimally invasive adenocarcinoma (MIA). While AIS and MIA predict an almost-certain recurrence-free survival if properly resected, an unsettling 20–40% of these nearly benign nodules undergo continued growth into invasive malignancies within a 4 year window if left alone.
