Zusammenfassung

In der Akutmedizin, insbesondere bei schweren Erkrankungen, die eine Behandlung auf der Intensivstation notwendig machen, ist in vielen Fällen eine empirische Initialtherapie, d. h. eine Behandlung mit Antiinfektiva vor Erregersicherung notwendig. Die Antiinfektivawähl in dieser Situation richtet sich nach dem erwarteten Erregerspektrum und der erwarteten Erregerempfindlichkeit (»kalkulierte«Therapie). Nach Erregersicherung kann und sollte in den meisten Fällen auf eine gezielte Therapie umgestellt werden. Dabei spielen die Aspekte Wirksamkeit, Nebenwirkungen, Resistenzentwicklung und Kosten eine Rolle.
