Neuropädiatrische Notfälle im Kindesalter

Zusammenfassung

Neurologische Notfälle sind häufig; die Ursachen sind vielfältig. Von allen Kindern, die akut auf die Intensivstation oder in die Notfallambulanz eingeliefert werden, sind 30 % (auch) neurologische Notfälle. Die Prognose ist von der Ursache und vom raschen Verstehen der Ätiologie abhängig. Die gezielte Anamnese und erste neurologische Untersuchungen können häufig zwischen den möglichen Ätiologien differenzieren. Der häufigste neurologische Notfall im Kindesalter ist der Krampfanfall, einschließlich Fieberkrämpfen bis zum Vorschulalter. In absteigender Häufigkeit sind in westlichen Industrienationen Schädel-Hirn-Traumata, Intoxikation, Hypoxien bzw. Ischämien (z. B. Ertrinkungsunfall) und Infektionen des Zentralnervensystems (ZNS) ursächlich. Der Status epilepticus muss schnell, spätestens nach 5 min, behandelt werden; beim Schlaganfall ist das spezialisierte Kinder-Stroke-Zentrum zu informieren, um das Zeitfenster für die Lysetherapie nicht zu gefährden.
