Brustimplantatassoziiertes Lymphom

Zusammenfassung

Hintergrund

Aufgrund fehlender Pflichtregister für Brustimplantate weltweit sind genaue Angaben zur Anzahl verwendeter Implantate nicht verfügbar. Der International Society of Aesthetic Plastic Surgery (ISAPS) zufolge wurden 2018 weltweit 1.841.098 Brustaugmentationen durchgeführt. In Deutschland kommen jährlich ca. 66.000 Brustimplantate zum Einsatz, davon ca. 7000 für Rekonstruktionen. Das brustimplantatassoziierte anaplastische großzellige Lymphom (BIA-ALCL), eine seltene Form des T‑Zell-Lymphoms, steht aktuell im Fokus der Diskussion über Qualität und Sicherheit von Brustimplantaten.

Fragestellung

Epidemiologie, Risikofaktoren, klinische Charakteristika und aktuelle Empfehlungen zur Diagnostik und Therapie des BIA-ALCLs.

Material und Methode

Auswertung der aktuellen Evidenz und der Empfehlungen der Fachgesellschaften.

Ergebnisse

Der Food and Drug Administration (FDA) liegen Angaben zu 573 BIA-ALCL-Fällen vor mit Berichten über 33 Todesfälle (Stand 07/2019). In Deutschland wurden bis 11/2019 22 Fälle an das Bundesinstitut für Arzneimittel und Medizinprodukte (BfArM) gemeldet. Als Hauptrisikofaktor wird derzeit die texturierte Implantatoberfläche diskutiert, die genaue Pathophysiologie ist unklar. Eine vollständige chirurgische Resektion ist die Therapie der Wahl bei Beschränkung auf die Kapsel, in höheren Stadien kann zusätzlich eine systemische Therapie analog der Empfehlungen für T‑Zell-Lymphome indiziert sein. Die Prognose ist mit einem 5‑Jahres-Überleben von 89 % gut.

Schlussfolgerungen

Das BIA-ALCL ist eine seltene, potenziell lebensbedrohliche Erkrankung, deren genaue Pathophysiologie bis jetzt ungeklärt ist. Es besteht sowohl eine Aufklärungs- als auch eine Meldepflicht, derzeit gibt es keine Empfehlungen für die Explantation bestimmter Implantate. Ein gesetzlich verpflichtendes Implantateregister einzuführen ist für 2021 vorgesehen.
