Chronische Prostatitis/chronisches Beckenschmerzsyndrom

Zusammenfassung

Die chronische Prostatitis/chronisches Beckenschmerzsyndrom (CP/CPPS) ist definiert als chronischer Schmerz oder Missempfindung im Beckenbereich für mindestens 3 Monate im letzten Halbjahr und weist eine häufige Assoziation mit Miktionsbeschwerden, psychosozialen Störungen und sexueller Dysfunktion auf. Weder Biomarker noch spezifische Testverfahren stehen für die eindeutige Diagnosestellung zur Verfügung. Die Basisdiagnostik bildet differenziert das individuelle Beschwerdebild des Patienten ab und schließt potenzielle Differentialdiagnosen für Beckenschmerzen aus. „Patient-reported outcome measures“ (PROMs) wie der Chronic Prostatitis Symptom Index (CPSI) sind validierte Instrumente zur objektiven Erfassung der Symptomatik und eignen sich für das initiale Assessment und weitere Therapiemonitoring. Das UPOINTS-Schema („urinary, psychosocial, organ specific, infection, neurologic/systemic, tenderness, sexual dysfunction“) bildet das individuelle Beschwerdebild der Patienten ab, weist den weiteren Weg der diagnostischen Abklärung auf und bestimmt das multimodale und personalisierte Therapiekonzept. Eine engmaschige urologische Begleitung der CP/CPPS-Patienten ist in der Regel notwendig, um insbesondere den unreflektierten Einsatz von Antibiotika bei wellenförmigen Beschwerden zu minimieren.
