Therapeutisches Vorgehen bei ANCA-assoziierten Vaskulitiden

Zusammenfassung

Hintergrund

Ergebnisse randomisierter multizentrischer Studien haben in der jüngsten Zeit die Entwicklung evidenzbasierter Empfehlungen zur Therapie der mit antineutrophilen zytoplasmatischen Antikörpern (ANCA) assoziierten Vaskulitiden (AAV) ermöglicht.

Methode

Es erfolgte eine selektive Literatursuche zu Studien und Leitlinien zu Therapie und Prognose der AAV.

Ergebnisse

Bei aktiver schwerer AAV führt die Kombination aus Prednisolon mit Cyclophosphamid (CYC) oder Rituximab (RTX) bei ca. 90 % der Patienten zu einem therapeutischen Ansprechen. Nach Erreichen einer Remission ist die Gabe von Azathioprin oder Methotrexat zur Remissionserhaltung über mindestens 2 bis 4 Jahre erforderlich. Trotz remissionserhaltender Therapie treten bei über 30 % der Patienten Rezidive auf. Bei einem Rezidiv ist RTX etwas wirksamer als CYC. Bei anhaltender oder progredienter Aktivität unter der Standardtherapie ist die Vorstellung in einem Zentrum sinnvoll. Trotz einer im Laufe der Jahre verbesserten Prognose ist die Frühmortalität insbesondere durch Infektionen noch erhöht.

Zusammenfassung

Die stadien- und aktivitätsadaptierte Therapiestrategie sowie die Versorgung in Zentren haben das Outcome der AAV in den vergangenen 3 Jahrzehnten zunehmend verbessert. Die erhöhte Frühmortalität und das Risiko von Rezidiven zeigen jedoch die Notwendigkeit einer weiteren Optimierung der Therapieprotokolle im Hinblick auf Substanzauswahl, Dosierung von Glukokortikoiden und Immunsuppressiva sowie der Dauer der Therapie.
