Pediatric fasting recommendations prior to anesthesia continue to evolve, and ongoing re-evaluation of preprocedural fasting guidelines is required. The scientific basis for many anesthesiology societies’ guidelines, including those of the Canadian Anesthesiologists’ Society (CAS), largely hinges on expert opinion with scant evidence to support improved outcomes. The rationale of fasting recommendations is based on the notion that the risk of aspiration, and its complications, will be diminished if the stomach is empty. It is difficult to rigorously test the belief that patient outcome is a function of fasting status as perioperative aspiration is a rare event (1 in 1,000 to 1 in 5,000) in elective pediatric anesthesia.
