Lebensqualität bei Epilepsie: Ein Überblick zum Forschungsstand – Teil 3: Korrelate und Determinanten von Lebensqualität

Zusammenfassung

Hintergrund

Seit den 1990er-Jahren ist in der Epileptologie Lebensqualität (Quality of Life [QOL]) bzw. gesundheitsbezogene Lebensqualität Gegenstand der klinischen Forschung. Dabei wird in einzelnen Studien meist nicht explizit erläutert, von welchem QOL-Konzept bzw. -Verständnis ausgegangen wird.

Ziele der Arbeit

In einem Überblick sollen die wichtigsten Ergebnisse der Forschung zur QOL bei Epilepsie aufgezeigt werden. In diesem 3. Teil geht es abschließend um den Forschungsschwerpunkt „QOL-Korrelate und Determinanten“, also um Studien, die analysierten, welche Variablen mit QOL assoziiert sind bzw. QOL beeinflussen.

Material und Methoden

Die Archive der einschlägigen Fachzeitschriften Epilepsia, Epilepsy & Behavior sowie Seizure seit Beginn der 1990er-Jahre wurden daraufhin untersucht, ob in Publikationen QOL thematisiert oder QOL-Daten erhoben worden waren. Auf repräsentative Studien wird verwiesen.

Ergebnisse

Bezüglich der QOL-Korrelate und Determinanten ist festzuhalten: Anfallskontrolle und Depressivität haben sich regelhaft als wichtige Determinanten von epilepsiespezifischer QOL erwiesen, wobei klinische Variablen wie Anfallskontrolle weniger QOL-Varianz aufklären als psychosoziale Variablen wie Depressivität.

Schlussfolgerung

Das QOL-Konzept ist nicht als bloßes, auf subjektiver Patienteneinschätzung basierendes Surrogat für das Behandlungsziel Anfallskontrolle zu verstehen. Es verweist vielmehr auf die Notwendigkeit, sich damit zu beschäftigen, ob weitere Krankheitsaspekte bzw. Komorbidität eine Belastung für den Patienten darstellen und ggf. therapeutischer Interventionen bedürfen.
