42. Allergie und Psychosomatik

Zusammenfassung

Psychosomatische Aspekte in der Allergologie sind zur Differenzialdiagnostik notwendig. Neben somatoformen Reaktionen – bei denen Patienten auf eine Allergie fixiert sind ohne positive Nachweise in der Allergiediagnostik – sind vor allem Angststörungen und Depressionen als Folge einer Allergie zu differenzieren und ggf. eine additive Indikation zur Psychotherapie zu stellen. Angstsymptome können anaphylaktoiden Symptomen sehr ähnlich sein und müssen von validierten Allergiereaktionen unterschieden werden. Neue psychoimmunologische Studien konnten den Einfluss von Neuromediatoren auf das allergische Entzündungsgeschehen sowohl in tierexperimentellen als auch in Studien am Menschen zeigen. Die neurogene Entzündung ist ein Erklärungsmodell, das die stressbedingte Exazerbation einer bestehenden Allergie erklärt. Psychotherapien sind in Deutschland gut etabliert; ergänzend kommen die psychosomatische Grundversorgung in der Allergologie selbst, die ambulante kassenärztliche Psychotherapie sowie stationäre Behandlungen in der Akut-Psychosomatik oder in psychosomatischen Reha-Kliniken für die allergologische Diagnostik und Therapie in Betracht.
