Guidewire-assisted endoscopic seton placement has been described mainly for simple, short, superficial perianal fistula in inflammatory bowel disease (IBD), particularly in fistulizing Crohn’s disease (CD) [
1]. Endoscopic seton placement has also been described for short ileal pouch-to-pouch fistula [
2]. Endoscopic seton placement can usually be done as a daycare procedure and can avoid surgical morbidity and prolonged hospital stay with early initiation of biologics in IBD. It can be quite cost-effective as well in resource-limited settings as compared to surgical seton placement [
