A 56-year-old man with a 15-year history of lupus erythematosus and well-controlled arterial hypertension developed progressively hypophonia, severe dysphagia, and weakness in the left shoulder. He was admitted to neurology for the worsening of the symptomatology. The neurological examination revealed loss of left-sided gag reflex, left soft palate paralysis with deviation of the uvula to the right, left-sided tongue atrophy with deviation to the left on protrusion, left-sided vocal cord palsy and mild atrophy of the left trapezius and sternocleidomastoid muscles. …
