We report on a case of fever of unknown origin (FUO) in which 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET)/CT assisted in depicting renal inflammation and eventually led to a histologically confirmed diagnosis of microscopic polyangiitis (MP). MP is an autoimmune disease characterized by systemic necrotizing, small-vessel vasculitis without clinical or pathological evidence of necrotizing granulomatous inflammation.
