A 59-year-old man with a history of metastatic pancreatic carcinoma attended for a
99mTc-HDP bone scan for lower back pain. The planar whole-body views showed no abnormal activity within the axial skeleton (
a). There was a wedge-shaped focus of intense tracer uptake within the right side of the head (
b). This was further characterised by lateral planar views. Eighteen days prior to the scan he had presented acutely with collapse. A CT scan showed an extensive cerebral infarct of the right middle cerebral artery territory (
