A 59-year-old man with a history of metastatic pancreatic carcinoma attended for a 99mTc-HDP bone scan for lower back pain. The planar whole-body views showed no abnormal activity within the axial skeleton (a). There was a wedge-shaped focus of intense tracer uptake within the right side of the head (b). This was further characterised by lateral planar views. Eighteen days prior to the scan he had presented acutely with collapse. A CT scan showed an extensive cerebral infarct of the right middle cerebral artery territory (c,d).
