Recurrent painful ophthalmoplegic neuropathy (RPON), previously known as ophthalmoplegic migraine, presents with recurrent unilateral headaches with associated ipsilateral ocular cranial nerve palsy. MRI features are present in 28% of cases, more often in children, and show nodular thickening and enhancement along the root entry zone (REZ) [1, 5]. Regression of contrast enhancement in the asymptomatic period is the discriminating feature in comparison to neoplastic processes or infection [1]. In most cases there is complete resolution of symptoms. We present two patients with atypical MRI features. …
