This month’s Journal Club will focus on ongoing clinical trials in three fields of neurology. First, oral fingolimod, a new immunomodulating agent that has direct effects on lymphocytes, was proposed for the treatment of relapsing multiple sclerosis (MS). This placebo-controlled, dose-finding study showed a significant reduction of the median total number of gadolinium-enhanced lesions on MRI and the annualized relapse rate after 6 months. Second, another clinical trial compared the efficacy of endarterectomy versus stenting in patients with symptomatic severe carotid stenosis. Unfortunately, this is again not the last word on this clinically very relevant topic. Third, new treatment strategies for mitochondrial disorders are proposed: platelet infusions or allogeneic stem cell transplantation, which reduces thymidine and deoxyuridine concentrations in the blood. …
