Hat das Implantatdesign einen Einfluss auf die Entstehung einer Periimplantitis?

Zusammenfassung

Eine der wichtigsten Langzeitkomplikationen von Implantaten ist die Periimplantitis, bei der die beiden klinischen Phänotypen der periimplantären Mukositis und der Periimplantitis unterschieden werden. Bei der periimplantären Mukositis betrifft die Entzündung das suprakrestale Weichgewebe, bei der Periimplantitis auch den ortsständigen Kieferknochen. Diese chronische Entzündung schreitet irreversibel voran und reduziert nach und nach das periimplantäre ortsständige Knochenangebot, wodurch es zum Implantatverlust kommen kann. Moderne leitliniengerechte Therapiekonzepte können das Fortschreiten der Periimplantitis stoppen bzw. verlangsamen und die lokalen Gewebeverhältnisse stabilisieren. Mikrofilm und bakterielle Kontamination der Implantatoberfläche spielen dabei eine wichtige Rolle, schlechte Mundhygiene gilt als prädisponierender Faktor. Zur Rolle des Implantatdesigns bei Periimplantitis gibt es Tendenzen in der Literatur, aber eine abschließende evidenzbasierte Beurteilung ist nicht möglich.
