Die AV-Knoten-Reentry-Tachykardie (AVNRT) ist die häufigste Form der paroxysmalen supraventrikulären Tachykardien (PSVT). Die AVNRT tritt klinisch als regelmäßige Tachykardie mit plötzlichem Beginn und abruptem Ende in Erscheinung. Die typischen Symptome umfassen Palpitationen, Angstzustände, Schwindel, thorakalen Druck, Dyspnoe sowie ein Klopfen im Hals und in der Brust. Im Anfalls-EKG findet sich in aller Regel eine regelmäßige Tachykardie mit schmalem QRS-Komplex und nicht klar identifizierbaren P-Wellen. Es sollte ein 12-Kanal-EKG der Rhythmusstörung aufgezeichnet werden. Differenzialdiagnostisch sind u. a. eine Sinustachykardie, eine atriale Tachykardie, Vorhofflattern und ein pseudoregularisiertes Vorhofflimmern zu berücksichtigen. Als Akuttherapie kommen vagale Manöver (z. B. Massage des Sinus caroticus) oder die Gabe von Adenosin infrage. Die Radiofrequenzablation ist die Therapie der Wahl zur kurativen Therapie der AVNRT.
