Das Leiomyom ist der häufigste Tumor des weiblichen Geschlechtsorgans. Bei 25–50 % der Myomträgerinnen treten Symptome wie pathologische Blutungen, eine blutungsbedingte Anämie, habituelle Aborte oder eine Sterilität auf. Medikamentöse Therapien basieren auf einer ovariellen Funktionssuppression durch GnRH-Analoga oder dem Einsatz selektiver Progesteron-Rezeptor-Modulatoren. Im Falle pathologischer Blutungen in der Perimenopause ist die Hysterektomie die adäquate Therapie. Neben der vaginalen Hysterektomie stehen die totale laparoskopische Hysterektomie, die laparoskopisch assistierte suprazervikale Hysterektomie oder die Hysterektomie per laparotomiam zu Verfügung. Konservative Verfahren sind die Myomenukleation per laparoskopiam oder laparotomiam, die hysteroskopische Myomabtragung, die Embolisation der A. uterina oder deren Koagulation, die Applikation von hochenergetischem Ultraschall oder die transzervikale ultraschallgesteuerte Hochfrequenzablation.
