Aufgrund des histopathologischen Erscheinungsbilds wird anhand der Zellgröße seit Langem zwischen nicht kleinzelligen (engl. non-small cell lung cancer, NSCLC) und kleinzelligen Lungenkarzinomen (engl. small cell lung cancer, SCLC) unterschieden. Demzufolge sind die Zellen des SCLC durch ihre geringe Größe charakterisiert und weisen eine hohe Kern-Plasma-Relation auf. Aufgrund der neuroendokrinen Charakteristika wird das kleinzellige Lungenkarzinom heute auch den neuroendokrinen Tumoren zugeordnet. Entsprechend ist immunhistochemisch in der Regel eine positive Reaktion der neuroendokrinen Marker CD56 und Synaptophysin nachweisbar.
