Mit der Revision der WHO-Klassifikation der Tumoren des Nervensystems im Jahr 2016 werden die diffusen Gliome des Erwachsenenalters nun nicht mehr nur nach der Histologie, sondern vor allem nach Isozitratdehydrogenase-(IDH-) und 1p19q-Status klassifiziert. Diese Revision hat erhebliche Konsequenzen für die nach Histologie und molekularem Profil stratifizierten Therapiestrategien bei Gliomen des Erwachsenenalters. Mit dem diffusen Mittelliniengliom wurde eine neue Entität definiert. Bei den meisten Therapiekonstellationen hat sich die kombinierte Chemoradiotherapie der jeweiligen Monotherapie als überlegen erwiesen. Neue Ansätze der zielgerichteten oder immunologischen Therapie haben bisher nur bei kleinen Subgruppen von Patienten Erfolge erzielt.
