Durch eine angeborene oder erworbene Störung des Abflusses der Lymphe über die mesenterialen Lymphgefäße kommt es zu einer Stauung von Chylomikronen und Lymphozyten, was zu einem Ödem der Darmwand führt. Wegen der mangelnden Abflussmöglichkeiten der Lymphe wird eiweißreiche Flüssigkeit in das Darmlumen abgepresst, verbunden mit einer Malabsorption von Fett, Cholesterin, Vitaminen und Mineralstoffen. Der enterale Verlust von Serumproteinen übersteigt die Syntheseleistung der Leber und führt zu schwerer Hypoproteinämie mit Hypalbuminämie verbunden mit Ödemen und Aszites. Verlust von Immunglobulin A (IgA) und IgG aus dem Serum führt zur sekundären Immundefizienz. Im peripheren Blutbild ist eine Lymphopenie auffällig. Bei seltenen angeborenen Defekten der Fettabsorption, wie bei Mutation des DAGT1-Gens kommt es zu einer abnormen Fettabsorption und Anreicherung von DAGT1-Substraten in der Mukosa mit Verlust von Protein in das Darmlumen. Bei CD55-Mangel kommt es zu schwerer proteinverlierender Enteropathie mit entzündlichen Veränderungen im Darm und thromboembolischen Komplikationen. Der Verlust von Plasmalemm-assozierter Vesikelprotein-RNA (PLVAP) führt zur einer Barrierestörung im Darm mit schwerem Eiweißverlust und Hypertriglyzeridämie.
