We present the case of a 31-year old male with a 2 year history of palpitations and early onset hypertension who presented for further evaluation. A screening echocardiogram demonstrated a 4 cm mass posterior to the left atrium. Cine MR studies were performed in long axis, short axis and four chamber views. Quantitative analysis of the LV and RV parameters were performed. Post contrast imaging and phase contrast sequences allowed viability and flow assessment. The MRI demonstrated a 4.9 × 4.4 × 3.9 cm homogenously enhancing mass with central flow voids involving the left atrium. The functional analysis was normal and there was no delayed enhancement in the myocardium. Followup
123I metaiodobenzylguanidine (MIBG) imaging of the chest and abdomen showed uptake in the mediastinum with persistence on delayed imaging (Fig.
