Gastrointestinal stromal tumors (GIST) of the second portion of the duodenum represent a rare clinical entity [
1]. This tumor location is technically challenging because of its proximity to the pancreas and the biliary/pancreatic ducts with the ampulla [
2]. GISTs arising from the major papilla have only been described in 12 case reports since 2004 [
3] and most authors advocate performing a pancreaticoduodenectomy (PD) for such lesions [
