14. Adipositaschirurgie, körperliche Aktivität und Trainingstherapie

Zusammenfassung

Körperliche Aktivität und Training stellen eine zentrale Säule des Therapiekonzepts bei Adipositas dar. Dies gilt auch für die morbide Adipositas, wobei der bariatrisch therapierte Patient sowohl prä- als auch postoperativ in mehrfacher Weise von einem Bewegungs- und Trainingsprogramm profitieren kann. Dazu zählen Wiedererlangung und Erhalt einer ausreichenden körperlichen Fitness mit Steigerung der gesundheitsbezogenen Lebensqualität, eine Unterstützung bei der Gewichtsreduktion, eine günstige Beeinflussung des kardiometabolen Risikoprofils sowie eine Verbesserung der glykämischen Kontrolle. Infolge des bei Patienten mit morbider Adipositas erhöhten Risikos für Komorbiditäten sollte vor Einleitung eines Bewegungstrainings geklärt werden, ob und in welchem Ausmaß die körperliche Belastbarkeit eingeschränkt ist. Bei der personalisierten Trainingsberatung sollte man sich insbesondere am Schweregrad des Übergewichts, dem kardiometabolen Risikoprofil, weiteren Komorbiditäten, der aktuellen körperlichen Aktivität und der aktuellen Leistungsfähigkeit orientieren. Sinnvoll ist auch die Berücksichtigung möglicher Vorerfahrungen mit körperlicher Aktivität und Sport, von sportlichen Neigungen und der Gegebenheit im Alltag des Patienten.
