Zusammenfassung

Fremd- und autoaggressives Verhalten kann Begleitsymptom unterschiedlicher psychischer Erkrankungen sein. In der Regel sollte auf aggressives Verhalten bei Kindern und Jugendlichen mit deeskalativen und pädagogischen bzw. psychotherapeutischen bzw. psychosozialen Maßnahmen reagiert werden. Eine pharmakotherapeutische Intervention kann allerdings gerade auch im Notfall indiziert sein. Dazu zählen insbesondere aggressives Verhalten bei Erregungszuständen, akute Exazerbationen psychotischer Störungen sowie bei Alkohol- bzw. Drogenintoxikationen und Selbst- oder Fremdgefährdung. Als pharmakologische Stoffgruppen stehen dazu Antipsychotika der zweiten/dritten Generation, niedrig- bis mittelpotente Antipsychotika der ersten Generation sowie in seltenen Ausnahmen auch Benzodiazepine zur Verfügung. Die Behandlungsdauer ist in der Regel kurzfristig. Bei Kindern und Jugendlichen mit Intelligenzminderung, bei Autismus-Spektrum- und Persönlichkeitsstörungen können auch längerfristige Anwendungen notwendig werden.
