A 24-year-old woman of Iraqi descent was referred to a nephrologist because of the incidental finding of chronic kidney disease stage 3 (estimated glomerular filtration rate 58 ml/min/1.73 m2, CKD-EPI) with microscopic hematuria and proteinuria (urinary protein-creatinine ratio of 4440 mg/g), without nephrotic syndrome. Her past medical history included bilateral sensorineural hearing loss since childhood and a family history of distant consanguinity. A kidney biopsy was performed (Fig. 1).-

Besides foam cells, histopathological examination showed two glomeruli out of four with focal segmental glomerulosclerosis, while the other two were unremarkable (Fig. 1D). Vessels were normal.
