Antimikrobielle Prothesenbeschichtungen

Zusammenfassung

Hintergrund

Antimikrobielle Beschichtungen besitzen großes Potenzial in der Therapie und Prävention periprothetischer Infektionen.

Ziel der Arbeit

Darstellung etablierter und neuartiger Konzepte antimikrobieller Beschichtungen.

Material und Methoden

Auswertung der Literatur und kritische Diskussion der publizierten Konzepte aus der Grundlagenforschung, tierexperimentellen Studien oder dem klinischen Einsatz.

Ergebnisse

Bis dato existiert eine Vielzahl an technischen Lösungen (antiadhäsive Oberflächen, bioaktive Oberflächen mit antimikrobiellen Eigenschaften, Oberflächen mit Freigabe antimikrobieller Substanzen, Nanostrukturen, bioaktive Oberflächen mit Beeinträchtigung der Biofilmbildung), die ihr Potenzial bereits in präklinischen Studien bewiesen haben. Nur wenige Lösungen – dies betrifft vor allem die bakterizide Wirkung von Silberbeschichtungen – haben die Marktreife erlangt. Belastbare Ergebnisse zur Biokompatibilität/Toxizität fehlen meist.

Diskussion

Trotz des großen Potenzials antimikrobieller Beschichtungen lassen die eingeschränkte Datenlage und der fehlende Nachweis der klinischen Wirksamkeit aus prospektiv kontrollierten Studien derzeit kein abschließendes Urteil zu. Innovative Konzepte müssen neben der uneingeschränkten Biokompatibilität im klinischen Alltag technisch umsetzbar sein.
