Multimodality therapy for gastric cancer has improved overall survival (OS) and disease-free survival (DFS) for patients with resectable disease compared with surgery alone, and these therapies appear to be particularly important for patients with postoperative complications (POCs) after resection.
1 Similar to other solid tumors, large retrospective studies have demonstrated that POCs impact oncologic outcomes in gastric cancer patients.
1 While this effect was previously attributed to delays in adjuvant therapy associated with POCs, alternative mechanisms include prolonged suppression of cell-mediated immunity and activation of microscopic residual disease (MRD) from the systemic inflammatory response to a POC.
2 Preclinical animal models have demonstrated that fluoropyrimidine-based therapy is effective in treating MRD of gastric cancer cell lines,
3 and based on this, the authors hypothesized that preoperative therapy would dampen the impact of POCs on survival in gastric cancer patients. …
