Bilateral spontaneous rupture of the long head of the biceps brachii tendon in rheumatoid arthritis

A fifty-eight-year old male farmer who had been diagnosed with rheumatoid arthritis (RA) 17 years ago was admitted to our department complaining of lumps on the anterior lateral aspect of his arms. The lumps were prominent when he flexed his forearms (Fig. 1). The first lump appeared on his right dominant arm 4 years ago and another one on the left 2 years later without any history of significant trauma. Since then he has given up farming as he complained about not being able to lift heavy weights. He has been under methotrexate (12.5 mg/week) therapy for the last 3 years and there has been no history of systemic or local corticosteroid administration.
