Zusammenfassung

Die primäre Diagnose des malignen Melanoms stützt sich v. a. auf die klinische Untersuchung, ergänzt durch die Dermatoskopie, und auf die histologische Untersuchung nach Exzisionsbiopsie. Das Staging bei Primärdiagnose dient der Ermittlung des Tumorstadiums nach AJCC (2009), das relevant für die Prognose und weitere Therapieentscheidungen ist. Die Rolle der Bildgebung bei der Primärdiagnose ohne Hinweis auf Metastasierung ist umstritten. Aktuell empfehlen die deutschen und europäischen Leitlinien bei einer Tumordicke von bis zu 1 mm keine weitere bildgebende Diagnostik, bei einer Tumordicke von >1 mm die Durchführung einer Sonographie der regionären Lymphknoten, eines Röntgen-Thorax und einer Abdomensonographie.
