Zusammenfassung

Die Fraktur des proximalen Humerus ist die dritthäufigste Fraktur des älteren Menschen nach hüftnahen Oberschenkelfrakturen und Radiusfrakturen mit steigender Inzidenz. Ursache dafür ist die altersbedingte Zunahme der Osteoporose. Im Gegensatz zum jungen sind es beim älteren Menschen meist Niedrigenergietraumen mit einfachen Stürzen, die als Traumamechanismus zur Verletzung führen. Mehrere Autoren stellten Systeme vor, um Humeruskopffrakturen zu klassifizieren.

Nach einer eingehenden klinischen Untersuchung der betroffenen Extremität mit Überprüfung von Durchblutung, Motorik und Sensibilität sollte auch auf Begleitverletzungen geachtet werden, v. a. im Bereich des Schultergürtels und des Thorax. Als Bildgebung wird eine a.-p.-Aufnahme und eine axiale Aufnahme der betroffenen Schulter gefordert.

Die Uneinigkeit in der Behandlung dieser Frakturen ist überaus groß und reicht von konservativer Behandlung über minimal osteosynthetische Verfahren, offenen winkelstabilen Verplattungen, Nagelungen bis hin zu prothetischen Versorgungen.
