Epileptic seizures are a common complication of stroke, especially in intracerebral haemorrhage (ICH). Post-stroke epilepsy (PSE) is typically defined as unprovoked seizures occurring beyond 7 days of stroke onset [
1], and is associated with increased mortality [
2,
3]. The risk of PSE after ICH is increased with younger age, larger haematoma volume, cortical involvement and presence of early seizures [
4], but it is unclear if radiological features such as irregular shape or heterogeneous density are associated with PSE. The role of primary prevention of PSE beyond the acute period of ICH remains to be established though a recent phase III randomised controlled trial showing that levetiracetam may be effective in preventing acute seizures [
5]. Predicting whether an ICH patient is at risk of developing PSE may inform clinical decision making in the treatment of ICH. …
