7. Endoskopische Therapie postoperativer biliärer Komplikationen

Zusammenfassung

Biliäre Komplikationen sind typische, aber relativ seltene Komplikationen – meist als Leckagen – nach Cholezystektomie und Leberteilresektionen. Komplikationen der Gallenwege treten jedoch bei etwa 30% aller Lebertransplantationen (LTx) auf. Sie sind damit die häufigste Komplikation nach LTx und resultieren in einer signifikanten Morbidität, Mortalität und Notwendigkeit der Retransplantation. Das Spektrum biliärer Komplikationen nach Lebertransplantation umfasst Gallelecks, Stenosen sowie Obstruktion durch „biliary cast“ und Konkremente. Die meisten Komplikationen können endoskopisch auch langfristig gut behandelt werden. Der Erfolg der endoskopischen Therapie hängt jedoch von der Art, der Lokalisation und dem Umfang der Schädigung sowie dem Zeitpunkt des Auftretens nach der Operation und der konsequenten Durchführung der endoskopischen Therapie ab.
