Therapie der idiopathischen Fazialisparese („Bell’s palsy“)

  • Die COVID-19-Erkrankung und die Impfung mit dem BNT162b2-mRNA-COVID-19-Impfstoff könnten mit einem gering erhöhten Erkrankungsrisiko verbunden sein [99, 102].
  • Die Behandlungsempfehlung mit Glukokortikoiden bleibt unverändert bestehen [69].
  • Eine ergänzende virostatische Therapie hat möglicherweise einen Zusatznutzen, v. a. bei initial schwerer betroffenen Patienten [38, 63].
  • Die Bedeutung der Differenzialdiagnose ist hervorzuheben, da 25–40 % aller Fazialisparesen nicht idiopathischer Genese sind.
  • Bei Schwangeren gelten die gleichen diagnostischen und therapeutischen Prinzipien. Die Glukokortikoidtherapie sollte allerdings unter stationären Bedingungen in einer spezialisierten geburtshilflichen Klinik vorgenommen werden.
  • Für die Beurteilung der Reinnervation nach einer Nervennaht wurde eine Ultraschallmethode vorgestellt [96]. Daneben können standardisierte Foto- und Videodokumentationen angewendet werden [31].
