Zusammenfassung

Bei der Penisfraktur rupturiert das Corpus cavernosum, in 10–20% auch das Corpus spongiosum und die Urethra. Die Kavernosografie und ggf. auch Urethrografie geben diagnostisch Aufschluss, der frühzeitige operative Verschluss der Tunika führt meist zu guten Langzeitergebnissen. Penetrierende Verletzungen des männlichen Genitale sind mögliche Folgen von Pfählung, Biss oder Schuss; die Diagnostik umfasst je nach Ausmaß verschiedene bildgebende Verfahren. Neben der chirurgischen Behandlung sind Tetanus-, ggf. Tollwutimpfung sowie eine geeignete Antibiose wichtig. Stumpfe Hodenverletzungen führen zur Ruptur oder Dislokation oder lediglich einem Skrotalhämatom. Sonografie, bei schwerer Verletzung auch CT oder MRT sind diagnostisch einzusetzen. Bei jedem Verdacht auf Ruptur ist eine operative Freilegung der Hoden indiziert. Sind beide Hoden betroffen oder bei vorliegendem Einzelhoden ist an eine Asservation von Hodengewebe zu denken.
