Zusammenfassung

Die interstitielle Nephritis führt häufig zu einem akuten Nierenversagen und wird oft spät oder nicht erkannt. Hauptursache sind Medikamente. Pathophysiologisch liegt meist eine Typ-IV-Hypersensitivitätsreaktion vor. Zudem gibt es seltenere Formen mit unklarer Ätiologie, die nicht selten unerkannt bleiben. Weil das pathophysiologische Verständnis oft fehlt, stehen kaum Erfolg versprechende Therapien zur Verfügung. Für manche Medikamente ist das Gesamtrisiko zwar gering, wegen häufiger Verordnungen sind die Nebenwirkungen im klinischen Alltag aber durchaus relevant. Zudem führen neue Ansätze wie die Immuntherapien zu Nebeneffekten, die nicht in vollem Umfang abzusehen sind. Bei vielen Erkrankungen erhöht das Auftreten eines akuten Nierenversagens die Mortalität und Morbidität. Eine potenziell irreversible chronische Niereninsuffizienz bewirkt weitere Komorbiditäten und schränkt die Lebensqualität ein. Die Therapie ist schwierig und meist empirisch.
