Hintergrund

Degenerative Rotatorenmanschettenrupturen kommen häufig vor. Dabei können insbesondere kleinere Rupturen asymptomatisch bleiben. Unbehandelt besteht eine Tendenz der Größenzunahme, so dass sich im Lauf der Zeit Beschwerden manifestieren können. Die Hauptsymptome sind Schmerzen und Funktionseinschränkungen. In einigen Fällen kann sich eine zu Beginn kleine Ruptur bis zu einer Massenruptur ausweiten.

Problem

Massenrupturen stellen ein Problem für den behandelnden Arzt dar. Bei fortgeschrittener Sehnendegeneration und Sehnenretraktion sowie Muskelverfettung sind einige dieser Rupturen irreparabel.

Therapie

Bei irreparablen Rupturen kann ein konservativer Therapieansatz gewählt werden. Es werden verschiedene Methoden beschrieben, die primär eine Schmerzlinderung und des Weiteren eine Funktionsverbesserung erzielen sollen. Die Therapie ist rein symptomatisch, ein Fortschreiten arthrotischer Prozesse kann dadurch nicht gestoppt werden. Bei Versagen der konservativen Therapie kommen chirurgische Verfahren in Betracht.
