Perioperatives Management bei alten Menschen mit Diabetes mellitus

Zusammenfassung

Multimorbidität, Polypharmazie und Malnutrition sind in der Versorgung des geriatrischen Patienten mit Diabetes mellitus besondere Herausforderungen, die im perioperativen Management berücksichtigt werden müssen. Die Stoffwechselreaktion auf Anästhesie, operativen Eingriff und postoperativen Verlauf ist deutlich verschieden von dem eines stoffwechselgesunden Gleichaltrigen.
Das optimale Glukosemanagement in der perioperativen Phase ist umstritten. Eine moderate und individuelle Einstellung des Blutzuckerspiegels vor der Operation wird empfohlen. Die engmaschige Korrektur einer Hyperglykämie durch intraoperative Insulingabe reduziert die Komplikationsrate. Das Risiko postoperativer Komplikationen und erhöhter Sterblichkeit ist abhängig von der Güte der akuten und der chronischen glykämischen Stoffwechsellage.
Prävalenz, Diagnose und Pathophysiologie der perioperativen Hyperglykämie sind Grundlage dieses Artikels, der gleichzeitig einen praktischen Überblick über die Behandlung von geriatrischen, chirurgischen Patienten mit Diabetes mellitus und Hyperglykämie geben soll.
