Psychologisch-psychiatrische Aspekte der Enuresis nocturna und der funktionellen Harninkontinenz

Zusammenfassung

Bei der Enuresis nocturna und der funktionellen Harninkontinenz spielen psychische Faktoren eine große Rolle. Es soll die Komorbidität von Enuresis/Harninkontinenz und klinisch psychischen Störungen, sowie subklinischen Symptomen dargestellt werden. In epidemiologischen, wie auch klinischen Studien zeigen 20–40% aller Kinder mit einem Einnässen nachts oder tags 2- bis 4-mal höher als nicht einnässende Kinder klinisch relevante psychische Störungen.

Das höchste Risiko für eine psychische Störung tragen Kinder mit einer sekundären Enuresis nocturna oder einer Harninkontinenz bei Miktionsaufschub, das geringste Risiko Kinder mit idiopathischen Dranginkontinenz und einer primären monosymptomatischen Enuresis nocturna.

Emotionale Störungen (wie Angst- und depressive Störungen) sind seltener als externalisiernde Störungen [wie (Aufmerksamkeitsdefizit-Hyperaktivitäts-Syndrom (ADHD) Hyperkinetisches Syndrom (HKS)]. Zusätzlich finden sich häufig ein hoher Leidensdruck und subklinische Symptome, die sich häufig mit einer erfolgreichen Therapie des Einnässens zurückbilden. Auch das Selbstwertgefühl kann ansteigen. Ein allgemeines Screening für psychische Symptome und Störungen wird empfohlen.
