Der artifizielle Sphinkter für die Inkontinenztherapie

Zusammenfassung

Während der letzten 40 Jahre wurde der artifizielle Sphinkter (AUS) erfolgreich implantiert. Dabei wurde die weitverbreitete Akzeptanz der Behandlungsmethode durch kontinuierliche technische Verbesserungen gewährleistet. Die Hauptindikation besteht nach wie vor bei der Inkontinenz nach Prostatektomie; in diesem Patientenkollektiv sind >90% mit dem Behandlungsergebnis zufrieden. Außerdem kann der AUS bei ausgewählten Patienten mit neurogener Blasenfunktionsstörung und bei angeborenen oder erworbenen anatomischen Fehlbildungen der Urethra eine Alternative zur Harnableitung darstellen. Trotz der geleisteten Fortschritte können Komplikationen wie Infektionen und Arrosion auftreten und in bis zu 20% der Fälle zu Revisionen oder Zweiteingriffen führen. Operative Expertise und die genaue Einhaltung der aseptischen Bedingungen sind daher obligate Voraussetzungen.

Die Hybrid-DRG-Verordnung soll dazu führen, dass mehr chirurgische Eingriffe ambulant durchgeführt werden, wie es in anderen Ländern schon länger üblich ist. Die gleiche Vergütung im ambulanten und stationären Sektor hatten Niedergelassene schon lange gefordert. Aber die Umsetzung bereitet ihnen doch Kopfzerbrechen.

Frauen mit Belastungsinkontinenz oder Organprolaps sind nach einer Netz-Operation keineswegs beschwerdefrei. Vielmehr scheint die Krankheitslast weiterhin hoch zu sein, sogar höher als von harninkontinenten Frauen, die sich nicht haben operieren lassen.

Adjuvante Immuntherapie verlängert Leben bei RCC

Nun gibt es auch Resultate zum Gesamtüberleben: Eine adjuvante Pembrolizumab-Therapie konnte in einer Phase-3-Studie das Leben von Menschen mit Nierenzellkarzinom deutlich verlängern. Die Sterberate war im Vergleich zu Placebo um 38% geringer.
