Endoprothetik bei Hallux rigidus

Zusammenfassung

Der Stellenwert der Endoprothetik in der operativen Behandlung von Hallux rigidus und anderen Zuständen mit Schmerz und Funktionseinschränkung des Großzehengrundgelenks (MTP1) ist im Gegensatz zur Arthrodese umstritten. Es fehlen vergleichbare Resultate prospektiver Langzeitstudien. Ein Trend zum Totalersatz des MTP1-Gelenks ergibt sich jedoch aus verbesserten biomechanischen und werkstoffkundlichen Erkenntnissen. Es werden einige zzt. markterhältliche Prothesentypen (Toefit-Plus™, Roto-glide™, ReFlexion™, Bio-Action™, Moje™) sowie die Erfahrungen der Autoren mit 30 Toefit-Plus-Totalprothesen vorgestellt. Es wurden 5 Komplikationen beobachtet (16,6%): 2 Prothesenluxationen, 3 Grundphalanxfissuren. In 14 Fällen mit mindestens 3-jähriger Verlaufszeit waren 3 Revisionseingriffe (21%) notwendig (Subluxationstendenz, persistierender Schmerz, Rigidusrezidiv). Einen sekundären Beweglichkeitsverlust zeigten 7 von 11 Fälle (63%) mit mindestens 4-jähriger Laufzeit. Eine aggressive erststrahlbelastende Nachbehandlung und einschränkende Indikationsstellung (Hallux rigidus, keine Experimente bei prioritärer Erststrahlinsuffizienz) werden weiterhin empfohlen.
