Zusammenfassung

Hintergrund

Die Behandlung der patellofemoralen Arthrose stellt hohe Anforderungen an den Orthopäden. Die exakte Analyse der zugrunde liegenden pathobiomechanischen Verhältnisse ist dabei die Basis jeder Therapieentscheidung.

Methoden

Sowohl Weichteilprozeduren wie die MPFL-Rekonstruktion (mediales patellofemorales Ligament) zur Stabilisierung als auch knöcherne Eingriffe zur Alignementoptimierung (Tuberositastransfer, Korrekturosteotomien) können in der Behandlung eine Rolle spielen. Bei weit vorangeschrittenen Patellofemoralarthrosen können diese Eingriffe auch in Kombination mit einem Teilgelenkersatz eingesetzt werden. Bei der Implantatwahl empfiehlt sich dabei die Verwendung anatomischer Prothesentypen, da mit diesen Komponenten die Anzahl an Begleitprozeduren reduziert werden kann.

Schlussfolgerung

Der Erfolg in der patellofemoralen Prothetik hängt in erster Linie vom Erkennen der biomechanischen Abweichungen ab. Gelingt deren Korrektur, kann das Risiko für ein Implantatversagen reduziert werden.
