Molekulare Prognosefaktoren und neue systemische Therapien bei Magenkarzinomen

Zusammenfassung

Die Abschätzung der Prognose von Magenkarzinompatienten sowie des Ansprechens auf adjuvante oder neoadjuvante Therapien mit Hilfe von molekularen Markern stellt einen viel versprechenden Ansatz dar, um die Prognoseabschätzung mittels klassischer, histopathologisch definierter Tumorstadien weiter zu verbessern. Voraussetzung für ihre Einführung in die klinische Routine bleibt jedoch die noch ausstehende Validierung an größeren Patientenzahlen in klinischen Studien sowie die Erarbeitung von verbindlichen Durchführungsrichtlinien für die Verfahren zu ihrer Bestimmung, um eine Vergleichbarkeit der Ergebnisse zu erreichen.
Viel versprechende Ergebnisse zu neuen biologisch zielgerichteten Medikamenten wurden aus Phase-I- und Phase-II-Studien berichtet. Phase-III-Studien, die letztendlich zur Zulassung neuer Medikamente führen könnten, werden mit Inhibitoren der epidermalen Wachstumsfaktorrezeptoren EGFR und Her-2neu (Cetuximab, Trastuzumab, Lapatinib), des epithelialen Zelladhäsionsmoleküls EpCAM (Catumaxomab) und des angiogen wirksamen endothelialen Wachstumsfaktors VEGF (Bevacizumab) durchgeführt. Ergebnisse dieser Phase-III-Studien sollten abgewartet werden, bis die entsprechenden Wirkstoffe Eingang in die klinische Routinetherapie finden.

Nun gibt es auch Resultate zum Gesamtüberleben: Eine adjuvante Pembrolizumab-Therapie konnte in einer Phase-3-Studie das Leben von Menschen mit Nierenzellkarzinom deutlich verlängern. Die Sterberate war im Vergleich zu Placebo um 38% geringer.

Das Risiko für Rezidiv oder Tod von Patienten und Patientinnen mit reseziertem ALK-positivem NSCLC ist unter einer adjuvanten Therapie mit dem Tyrosinkinase-Inhibitor Alectinib signifikant geringer als unter platinbasierter Chemotherapie.

Patienten, die zur Behandlung ihres Prostatakarzinoms eine Androgendeprivationstherapie erhalten, entwickeln nicht selten eine Anämie. Wer ältere Patienten internistisch mitbetreut, sollte auf diese Nebenwirkung achten.
