Transanal total mesorectal excision (taTME) has emerged as a minimally invasive technique for curative-intent resection of rectal cancer [
1]. In particular, using the transanal minimally invasive surgery (TAMIS) technique [
2], the ability to properly approach the distal mesorectal plane and achieve an oncologically adequate resection is often enhanced. However, there are important areas in which the taTME technique could introduce procedure-specific morbidity [
3], especially injury to the male urethra and, with transanal extraction, conduit ischemia. Furthermore, the technique for taTME with intersphincteric dissection (ISR) is not standardized and is rarely described in the published literature. …
