Psychoonkologische Unterstützung von Patienten und Angehörigen in der Terminalphase

Zusammenfassung

Neuere Untersuchungen beschäftigen sich mit unterschiedlichen Aspekten psychischer Reaktionen auf die Diagnose einer fortschreitenden und terminalen Krebserkrankung bei Patienten und ihren Angehörigen. Dazu zählen Phänomene des Bewusstseins über die Unheilbarkeit der Erkrankung und die verkürzte Lebenserwartung („prognostic awareness“), Todesangst sowie Verlust an Lebenssinn und Lebensmut (Demoralisierung). Dieser Beitrag gibt einen Überblick über ausgewählte Belastungen von Patienten und ihren Angehörigen in der palliativen Versorgungssituation und stellt den Stand der psychoonkologischen Interventionsforschung vor.

Wenn bei Mammakarzinomen durch eine neoadjuvante Chemotherapie ein Downstaging von nodal-positiv zu nodal-negativ gelingt, scheint es auch ohne Axilladissektion nur selten zu axillären Rezidiven zu kommen.
