Zusammenfassung

Die Fortschritte in der Peri- und Neonatologie innerhalb der letzten Jahrzehnte haben zu einem Anstieg der Überlebensraten sehr unreifer Frühgeborener, d. h. Frühgeborener eines Gestationsalters von weniger als 26 SSW oder eines Geburtsgewichts von weniger als 750 g, geführt. Innerhalb der letzten 5 Jahre publizierte Studien berichteten Überlebensraten extrem unreifer Frühgeborener zwischen 33% und 62%. Die beschriebenen Anteile der wesentlichen Komplikationen der Frühgeburtlichkeit variieren ebenfalls beträchtlich. Die bronchopulmonale Dysplasie ist am häufigsten (Prävalenz bei überlebenden Kindern eines Gestationsalters von weniger als 26 SSW: 35–80%), gefolgt von schweren intraventrikulären Hirnblutungen und der schweren Frühgeborenenretinopathie (Prävalenz bei überlebenden Kindern eines Gestationsalters von weniger als 26 SSW: 17–36% bzw. 9–18%). Die Unterschiede in den Überlebens- und Komplikationsraten erklären sich u. a. durch Differenzen in der Definition der Untergrenze der aktiven Behandlung Frühgeborener und durch ein unterschiedliches Studiendesign.
