Zusammenfassung

Zunächst werden die Prinzipien der experimentellen Schmerzmessung dargestellt und ihre Rolle im klinischen Feld diskutiert. Anders als beim klinischen Schmerz kann beim experimentellen Schmerz sowohl die Reiz- wie die Reaktionsseite quantifiziert werden. Die wesentlichen Schmerzinduktionstechniken und Schmerzreaktionsparameter sowie die Nutzung des Quantitative Sensory Testings (QST) werden vorgestellt. Ein besonderer Abschnitt befasst sich mit der Schmerzsensibilität im aktivierten Schmerzsystem. Dem folgen einige besondere Probleme der biomedizinischen Diagnostik und ausführlich die wesentlichen Bereiche der psychosozialen klinischen Diagnostik. Hierzu gehören Verfahren zur Erhebung des Schmerzerlebens sowie zur Beschreibung der kognitiv-emotionalen, behavioralen und sozialen Dimensionen des chronischen Schmerzes, soweit sie der klinischen Syndrombeschreibung bzw. -analyse und Therapieevaluation dienen. Empfehlungen für eine Standarddiagnostik werden gegeben.
