Neue Definition der Sepsis

Implikationen für Diagnostik und Therapieprinzipien

Zusammenfassung

Bis vor kurzem beruhten die Sepsiskriterien auf der Vorstellung, dass die Sepsis in erster Linie durch die inflammatorische Reaktion des Körpers auf eine Infektion verursacht war, was nicht dem aktuellen Wissen über die Pathophysiologie der Sepsis entspricht. Entsprechend wird die Sepsis nun als eine lebensbedrohliche Organdysfunktion aufgrund einer fehlregulierten Antwort des Körpers auf eine Infektion definiert. Ein septischer Schock liegt vor, wenn ein Zustand persistierender Hypotension mit kontinuierlichem Vasopressorbedarf und Serumlaktatspiegeln von >2 mmol/l trotz ausreichender Volumen- bzw. Flüssigkeitsgabe zu verzeichnen ist. Diese neuen Definitionen werden hier mit ihren Konsequenzen für die Diagnose der Sepsis diskutiert. Auch werden in diesem Übersichtsartikel die aktuellen Kontroversen zu den wichtigsten Aspekten der Sepsistherapie behandelt.
